Our goal is to help you achieve optimal health so you can manage better at home and be involved in your community.

You may receive the following services through the Home and Community Care Program for Younger People:

  • ·       exercise physiology
  • ·       exercise group programs
  • ·       occupational therapy
  • ·       podiatry
  • ·       diabetes education
  • ·       nursing
  • ·       linkages (we help you navigate the healthcare and disability support system).

We then deliver the necessary services to help you be as independent as possible.

How do I access this program?

You may be eligible for the Home and Community Care Program for Younger People if you:

  • ·       live in Victoria
  • ·       are younger than 65, or identify as First Nations and are younger than 50
  • ·       have a chronic illness, mental health issue, disability, or a condition that stops you from doing everyday activities and taking part in your community.
