Access to Personal Health Records

Latrobe Community Health Service is not subject to the Freedom of Information Act 1982. Requests for personal health information by clients or customers of Latrobe Community Health Service are now regulated by the Health Records Act 2001.

Accessing Records

Requests for information must be in writing and include:-
·     Your name and address;
·     Proof of identity or evidence of your authority to act on another person’s behalf;
·     Identify sufficiently the health information you are seeking access to; and
·     Specify the form of access to the information you require eg. copy of records, inspection
